Abstract

Elderly people and individuals with disabilities face difficulties in managing medications due to polypharmacy and cognitive decline, which leads to forgetfulness and dosage errors. About 20% of patients with chronic diseases do not follow the prescribed treatment (SciELO, 2020), and 6% of hospital admissions are linked to incorrect medication use (ANVISA, 2019). These factors compromise safety, autonomy, and quality of life.
